"Follow the worms" is a euphemism for, as shown clearly in the video, the insanity of following the ideals of fascism, right wing bigotry and violence etc. The main character has gone insane and is hallucinating that he is a fascist leader inciting violence against innocents.
In 1979 this was a reference to the fascism of the 1940's (Roger Waters' father died in WW2 as a British soldier fighting against fascist Germany). Sadly it now seems all too contemporary.
The very end of the album makes it clear the good people trying to get through to the mentally ill main character are "bleeding hearts and artists". "Bleeding heart" was a term used at the time for liberals, a reference to their compassion, in contrast to the horrors of the right shown in the Waiting for the Worms scene.

Nice. I can't remember if I saw this YTMND before, or it was just too long ago, but the song does sound familiar.
