That would be because nations like North Korea, China and Iran can actually fight back. Certainly China, having the largest number of armed forces in the world And the other two having threatening weapons. You can't simply solve this problem by unloading a nuclear Christmas present on these countries either.
I don't know of any reasons Saudi Arabia or Iran violates human rights other than perhaps through debatable Sharia law.
And action was not formally taken against Indonesia's aggression, but military force was moved in East Timor on two occasions to avert further troubles, without actually causing war.
Consider two other reasons,
the fact that the United States has to face up to anti-war protests for any of these potential wars like they have for every war since Korea.
and of course, the fact that with each war, America will lose popularity in the world, as they gain the reputation for being a warmongering nation.
Those reasons mean that a war cannot be begun without an appropriate event to spark its declaration. For some people, human rights violations would not justify a war and it wouldn't go ahead.
And by the way, just because a nation is militarily powerful, doesn't mean it is a threat.
